Gold Circle and Silver Circle Nomination Form

The Gold Circle honors those who have made significant contributions to the broadcast or cable industries in the National Capital and Chesapeake Bay region over the span of 50 years, either in a performing, creative, technical or administrative role. The Silver Circle honors those who have made significant contributions over the span of at least 25 years.

Candidates or their sponsors should submit the following information for evaluation and selection by the Board of Governors of the National Capital Chesapeake Bay Chapter of the National Academy of Television Arts and Sciences.

If your candidate is chosen, you will be asked to arrange for an edited three-minute video that will be part of the awards ceremony.
